What do I upload here?

Your facility's MDS Coordinator will upload the exact same file that is transmitted to CMS to track the patients in your building.

What if I upload the wrong file?

Don't worry, SNFQAPI will tell you if you're uploading the wrong file. Remember this is the same file you transmit to CMS. For further assistance on generating your MDS batch file please call us at (844) 763-7274 ext. 2.
